Evaluation Process

Our evaluation process begins with calling our facility (312-642-4300) or using our website to fill out an initial intake form and developmental summary.  These two forms give our therapists a well-rounded picture of your child before meeting for the first time.  Additional forms can be completed at your first visit, or if you prefer you can print and complete registration forms prior to your initial appointment.

Your First Appointment

We want to make the first experience for you and your child successful.  To help make the transition smooth we recommend you review the check list below prior to your first appointment.  Plan to arrive a few minutes early to settle into the waiting room and complete registration forms if you haven’t already done so. 

 CHECKLIST

    •    Intake Form and Developmental Summary completed
    •    Doctor’s prescription obtained
    •    Check directions and parking information
    •    Arrive 10 minutes early to settle and complete registration forms
